Our Continuity of Support Programme provides assistance to individuals to find the most appropriate support when transitioning from the Home and Commnity Care Programme (HACC) in Western Australia.
People who do not meet the NDIS residency requirements and are seeking access to State-funded disability-related supports may be eligible for Department of Communities Continuity of Support Programme.
